Hartford HealthCare is hiring a Case Coordinator - RN for St. Vincent's Medical Center in Bridgeport, CT. The role focuses on assessing patients, coordinating care, and developing discharge plans in collaboration with the Health Care Team.

You will advocate for patient needs, educate families and professionals, and maintain thorough documentation while working in a full-time day shift with rotating weekends/holidays.

Qualifications

• Completion of a formal nursing program is required.
• Associate degree hires must obtain a BSN within 6 years.
• 3 years of acute-care nursing experience.
• Experience in case management, home care, and/or managed care preferred.
• Licensed Registered Nurse in CT; BLS certification preferred.

Responsibilities

• Assess patient population with the Health Care Team for appropriate level of care.
• Develop and implement discharge planning in collaboration with the Health Care Team.
• Collaborate daily with direct care providers to assess in-hospital stays.
• Model quality patient/customer service by maintaining collaborative relationships with caregivers.
• Provide ongoing assessment and develop interventions and treatment strategies.
• Advocate for the needs and interests of patients, including system changes for improved access to care.
• Educate patients, families, and professionals regarding disease prevention and health maintenance.
• Maintain timely documentation of case management services.
• Identify discharge delays and develop countermeasures for potential barriers.
• Collaborate with providers to ensure appropriate patient placements throughout hospitalization.

Experience

• 3 years of acute-care setting, diversified clinical nursing experience.
• Case management, home care, and/or managed care experience preferred.

Licenses & Certifications

• Licensed Registered Nurse credentialed from the Connecticut Department of Public Health.
• Certified BLS Provider credentialed from the American Heart Association (AHA) or American Red Cross preferred.
• Case Management Certification preferred.
