PALS, LLC has been providing quality Supported Living Services to adults with developmental disabilities for over 25 years. Established 1998, PALS, LLC specializes in personally designed services that are tailored fit to suit the individuals needs. Located in Whittier and Indio, California.

Role Description

This is a full-time on-site role for a Case Coordinator at PALS SLS located in Coachella, CA. The Case Coordinator will be responsible for:

  • Training and coordination of assigned staff
  • Planning managing, coordinating, and evaluating assigned staff efforts to achieve the programs objectives and consumer outcomes
  • Coordinating with the State of California agencies regarding the implementation of consumer Individual Service Plan objectives for which PALS is responsible
  • Assessing the program's effectiveness in achieving consumer Individual Service Plans objectives
  • Identifying barriers to consumer success and making plans to overcome those barriers
  • Facilitate staff, consumer and circle of support interactions to facilitate scheduling options for consumers instructional programs and activities
  • Direct support staff to develop weekly work schedules to meet consumer needs
  • Monitor the number of hours of service provided to each consumer on an assigned caseload

Minimum Qualifications

  • Bachelor's Degree
  • Three years of experience in a human services delivery system, including at least one year in a comparable program or a bachelors degree in a human service related field
  • Proficient writing skills
  • Proficient computer skills, including but not limited to Microsoft word
  • The demonstrated ability to provide staff training, supervision, and planning
  • Must have a valid C.A. drivers license, auto insurance, reliable and safe vehicle
  • Proficient in Spanish (required)
