Position Summary

Coordinates and manages comprehensive care by assessing individual needs, creating personalized care plans, and facilitating services across healthcare providers. Acts as a patient advocate to ensure clear communication and access to appropriate resources.

  • Required Qualifications

  • Active Independent Behavioral Health License in your state of residence.

  • Reside in Mountain Time Zone, or be able and willing to work Mountain Time Zone Hours of 8-430 MT.

  • Minimum of 3 years of post-master’s clinical experience in behavioral health.

  • Strong understanding of mental health and substance use disorders.

  • Proficiency in technology, including Electronic Medical Records (EMR), Microsoft Office Suite, and general computer applications.

  • Dedicated, private home workspace with reliable high-speed internet.

  • Remote position: Must work exclusively from your primary home residence; working from alternate locations is not permitted.


Preferred Qualifications

  • Experience incase managementanddischarge planning.

  • Familiarity withmanaged care systemsand processes.

  • Previousremote or work-from-home experience.

  • Skilled inmotivational interviewingtechniques to support patient engagement and behavior change.



Education

  • Master’s degree inBehavioral Health,Social Work, or a closely related field.

Pay Range

The typical pay range for this role is:

$60,522.00 - $129,615.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
