Job summary

Pyramid Healthcare, Inc. is seeking a Case Coordinator to manage and coordinate care for clients and families in residential treatment, including admission, aftercare planning, and coordination with the clinical team.

The role involves scheduling, communication with referral sources, and collaboration with treatment staff to enhance engagement in care. Shift is Monday–Friday, 8:00am–4:00pm, with pay between $15.50 and $17 per hour.

Qualifications

• High School Diploma or equivalent required.
• One year of experience in a drug and alcohol or mental health setting is preferred.
• Driver’s License is preferred.
• CPR/BLS certification is preferred.

Responsibilities

• Coordinate client engagement by managing appointments, aftercare plans, schedules, and meetings with the treatment team.
• Maintain communication with referral sources and outside providers, documenting contacts from admission through treatment.
• Provide case coordination including arranging transportation and individual coordination sessions.
• Assist in developing aftercare plans, facilitating referrals, and addressing external provider needs.
• Support clinical services by assisting in group facilitation and client engagement weekly.
• Collaborate with the treatment team and maintain professional communication with counselors and team members.
• Document client, family, and provider contacts in Carelogic according to program timelines.
• Work with the client navigation team on client aftercare needs and onsite EOS support.
• Participate in mandatory corporate and program training sessions and engage in professional development opportunities.

Summary:

Case Coordinators are responsible for managing and coordinating care for clients and families in residential treatment. This includes individual and family case management services to enhance engagement in treatment, facilitating educational and skill development groups in collaboration with the clinical team, and overseeing admission, aftercare/discharge planning, and coordination with external providers.

Shift:

Monday - Friday 8:00am - 4:00pm

Pay:

$15.50 - $17/hr
