Job summary

Hidden Garden Medical Case Management in Baltimore, MD seeks a Case Assistant to support the Ryan White Clinical Quality Manager by monitoring clients' adherence to HIV primary care and related services, arranging reminders and transportation, and assisting with rescheduling as needed.

Responsibilities include updating eligibility for Ryan White services, making outreach calls, conducting home visits for lost-to-care clients, and coordinating access to behavioral health and other specialty

Qualifications

• Bachelor’s Degree with two years’ experience in a social service environment.
• Sensitivity to HIV/AIDS related issues and African-American and other minority community issues.
• Awareness of local AIDS services and resource providers at the public and community levels.
