Job summary

Mosaicms in Corvallis, Oregon seeks a compassionate Caregiver for its memory care community. You will provide direct resident care, build meaningful connections, and help residents live each day with dignity and joy.

Ideal candidates have a high school diploma and prior caregiving experience, with a preference for memory care knowledge. The role involves flexible evening/weekend shifts and collaboration with a caring team to support daily living activities.

Qualifications

• High school diploma or equivalent.
• Prior caregiving experience preferred.
• Experience in memory care preferred.

Responsibilities

• Provide hands-on care including bathing, grooming, dressing, toileting, and mobility.
• Assist with meals, snacks, hydration, and feeding as needed.
• Support residents with ambulation and range-of-motion activities.
• Monitor residents' comfort levels and address needs promptly.
• Document all care and observations per Mosaic policies and regulatory requirements.
• Report changes to the Caregiver Coordinator or Executive Director.

Key Responsibilities

• Resident Care & Support
• Provide compassionate, hands-on care to residents, including assistance with bathing, grooming, dressing, toileting, and mobility.
• Assist residents with meals, snacks, hydration, and feeding as needed.
• Support residents with ambulation, positioning, and range-of-motion activities.
• Monitor residents’ comfort levels and promptly address needs and concerns.
• Health Monitoring & Documentation
• Monitor and record vital signs, weight, and fluid intake/output accurately.
• Report any changes in residents’ physical, emotional, or cognitive status to the Caregiver Coordinator or Executive Director.
• Document all care, observations, and resident interactions in compliance with Mosaic policies and regulatory requirements.
• Resident Engagement & Activities

Qualifications & Skills

• High school diploma or equivalent required.
• Prior caregiving experience preferred; healthcare or Memory care experience strongly valued.
• Ability to read, write, and communicate effectively in English.
• Must demonstrate patience, empathy, and a genuine desire to support seniors.
• Knowledge of assisted living, memory care, and related regulations preferred.
• Ability to work collaboratively in a fast-paced, team-oriented environment.
• Flexibility to work evenings, weekends, and holidays as needed.
