Job summary

Clarity Care Inc in Fond du Lac, WI is seeking an Entry level Caregiver to join our compassionate team. This role supports group home residents with daily living activities and personal care, with no prior experience required and starting pay up to $18/hr based on experience.

You will help with meals, housekeeping, transportation duties, and community activities while following the individual service plan and safety guidelines.

Qualifications

• Must possess a high school diploma/equivalent and/or related work/life experience.
• Must be at least 18 years of age.
• Must possess a valid WI driver's license and provide proof of auto insurance.
• Must pass a WI Caregiver Background Check including DOJ checks.

Responsibilities

• Hands on caregiving. Provide direct care support to members with disabilities, assisting with activities of daily living; bathing, grooming, toileting, and hygiene as per the service plan; comfortable with mobility devices and safe transfer/ambulation techniques.
• Teaches and guides members toward goals through self-direction, decision making and goal setting.
• Completes all applicable documentation related to tasks assigned.
• Welcoming home: maintain a safe, sanitary and welcoming group home via housekeeping and laundry.
• Prepares and serves meals following general guidelines and dietary needs.
• Assists, escorts and provides support during social and recreational activities.

The Caregiver assists with the day-to-day operations of the group home. They carry out responsibilities in the following functional areas: consumer rights, personal cares, daily living activities, and regulatory compliance.

Key Responsibilities

• Hands on Caregiving. Provides direct care support to our members with disabilities by assisting with activities of daily living; bathing, grooming, toileting, and hygiene as outlined by the individual service plan. Be comfortable using mobility devices and practice safe transfer and ambulation techniques like in assisted living facilities.
• Teaches and Guides members in achieving hopes, dreams, goals and objectives through self-direction, decision making, and goal setting.
• Completes all applicable documentation related to tasks assigned.
• Welcoming Home. Maintains a safe, sanitary and welcoming group home by performing housekeeping (vacuuming, emptying trash, cleaning bathroom, dusting, bed making and changing linens); and laundry.
• Prepares and Serves Meals by following general nutritional guidelines, menus, individual dietary restrictions, needs and preferences to ensure consumer health.
• Community Activities. Assists, escorts, and provides physical and other types of assistance needed by members during social and recreational activities.

EDUCATION/QUALIFICATIONS

• Must possess a high school diploma/equivalent and/or related work/life experience.
• Must be at least 18 years of age.
• Must possess a valid WI driver’s license, and ability to meet agency driving standards. Must provide proof of auto insurance.
• Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks, and/or that of previous states of residence
