Clarity Care, Inc. is hiring for an Activity Support Provider to assist with day services and community activities in Wisconsin. Starting pay is up to $18/hr based on experience, with no prior experience or certifications required.

Qualifications

• Must possess a high school diploma/equivalent.
• Must be at least 18 years of age and have a valid WI driver’s license with proof of auto insurance.
• Must pass a periodic WI Caregiver Background Check including DOJ Criminal Record Checks or those of previous states of residence.

Responsibilities

• Support members in community and recreational activities.
• Assist members with physical, social, and personal needs.
• Guide members in achieving personal goals and decision-making.
• Create engaging activities tailored to member interests.

The Activity Support Provider assists with the day-to-day operations of the Day Services Program. They carry out responsibilities in the following functional areas: goal setting, independence, community involvement, consumer rights, personal cares, daily living activities, and regulatory compliance.
