Old Colony Elder Services, Inc. is seeking a dedicated Family Caregiver Specialist to assess and coordinate community-based services for caregivers of elders and others, including grandparents raising grandchildren.

The role supports clients in staying active and engaged in their communities. The position offers a hybrid work environment with a 35-hour workweek, opportunities for professional development, and generous benefits.

Qualificações

• Bachelor's degree in social work or related field.
• 1+ year of relevant experience in a community or elder service agency.
• Proficient with Microsoft Office Suite or related software.
• Must have personal computer with secured internet access at home.

Responsabilidades

• Maintain a caregiver caseload and complete assessments, care plans, and ongoing support.
• Facilitate or support caregiver groups and administer respite-related financial scholarships.
• Document services, outcomes, and program activity according to guidelines.
• Participate in outreach, educational events, and community presentations to promote OCES services.
• Build and maintain partnerships with community organizations to improve communication and resources.
• Facilitate evidence-based and other community trainings.
• Follow program requirements for FCSP and Healthy Living initiatives.
• Complete required reporting and contribute to program evaluation.

We are seeking a dedicated & caring Family Caregiver Specialist who evaluate and assess the need for supportive and community-based services to support caregivers who are caring for elders and others, including grandparents raising grandchildren, and maintaining their care recipient in the community.

What you'll do:

• Maintain a caregiver caseload and complete assessments, care plans, and ongoing support.
• Facilitate or support caregiver groups and administer respite-related financial scholarships.
• Document services, outcomes, and program activity according to guidelines.
• Participate in outreach, educational events, and community presentations to promote OCES services.
• Build and maintain strong partnerships with community organizations, serving as a liaison to improve communication, collaboration, and the development of caregiver and wellness resources.
• Facilitate evidence-based and other community trainings.
• Follow business rules and program requirements for FCSP and Healthy Living initiatives.
• Complete required reporting and contribute to program evaluation.
