Job summary

Addus HomeCare is seeking a dedicated Caregiver Coach to guide and support primary caregivers of clients enrolled in Medicaid Waiver programs. You will assess needs, develop coaching plans, and deliver training to improve participant care and caregiver well-being.

Responsibilities include in-home or virtual coaching, scheduling at least two visits per quarter, documenting activities, and providing ongoing education.

Qualifications

• Minimum education includes high school diploma or equivalent; bachelor's degree preferred.
• Experience in home care, case management, behavioral health, or direct support is valued.
• Excellent oral and written communication skills required.

Responsibilities

• Provide guidance, education, and support to primary caregivers of clients in Medicaid Waiver programs.
• Develop and deliver caregiver coaching plans and training materials.
• Conduct initial consultations and ongoing coaching based on assessments.
• Maintain caregiver engagement with regular visits and documentation in records.
