Job summary

Hartford HealthCare Independence at Home seeks a Homemaker Caregiver to support elderly and disabled clients in their homes across Connecticut. You will assist with personal hygiene, meals, light housekeeping, and mobility, reporting changes to the nurse or supervisor.

Ideal candidates have patient care experience, a valid driver’s license, reliable transportation, and a compassionate, professional demeanor. Two years of continuous work history and willingness to learn are preferred.

Qualifications

• Two years of continuous work experience.
• One year of previous patient care experience; specific experience as designated by work area may be required.
• New Grad CNAs are welcome.
• Must have a current and valid Driver’s License in good standing along with reliable transportation and proof of auto insurance.

Responsibilities

• Provide personal hygiene and grooming care to clients; assistance with toileting, ambulation, etc.
• Provide meals for clients' nutritional needs.
• Report changes in clients' condition to nurse, supervisor, or client care coordinator.
• Perform routine housekeeping and home management duties.
• Record each client's care on care plan and submit care plan in a timely manner.
• Assist with providing emotional support for clients and their families.
• Assist with routine therapy exercises.
• Participate in case conferences as requested.
• Attend required in-service education sessions.
• Use computer software and smartphone for required education.
• Represent the company in a positive and professional manner through a high level of customer service.
• Think critically without direct supervision and the ability to escape when appropriate.
