Job summary

Hartford HealthCare at Home is seeking a Hospice Aide to provide compassionate in-home care within the Bridgeport area. You will follow the care plan, assist with daily activities, and support patients and families in their homes.

Qualifications include a High School diploma and a Connecticut CNA certification. Six months of home care experience is preferred. Growth opportunities are available within Hartford HealthCare's extensive network.

Qualifications

• Education: High School diploma or equivalent; CNA certification required.
• Connecticut Department of Public Health and Addiction Services CNA certification required.
• Six months experience as Home Care Aide/CNA preferred.

Responsibilities

• Provides personal care and housekeeping assistance to clients in their homes; follows the established plan of care under the direction of the client care coordinator.
• Provides personal hygiene and grooming care to clients.
• Provides meals for clients' nutritional needs.
• Reports changes in clients' condition to client care coordinator or supervisor.
• Performs routine housekeeping and home management duties.
• Records each client's care on care plan and submits care plan in a timely manner.
• Responds sensitively to the multiple, complex needs of a terminal client which includes providing end of life care.
• Assists with routine therapy exercises.
• Participates in case conferences as requested.
