Mosaic Management in Forest Grove, Oregon, seeks a care-focused Caregiver to support residents in an assisted living and memory care setting. You will assist with daily living activities, meal preparation, and health monitoring to help residents maintain independence with dignity.

Qualifications

• High school diploma or equivalent is required.
• Prior caregiving experience or healthcare/long-term care experience is valued.
• Ability to read, write, and communicate effectively in English.
• Must demonstrate patience, empathy and a genuine desire to support seniors.
• Knowledge of assisted living, memory care and related regulations is preferred.
• Ability to work collaboratively in a fast-paced, team-oriented environment.
• Flexibility to work evenings, weekends and holidays as needed.

Responsibilities

• Provide compassionate, hands-on care to residents, including assistance with bathing, grooming, dressing, toileting, and mobility.
• Assist residents with meals, snacks, hydration, and feeding as needed.
• Support residents with ambulation, positioning, and range-of-movement activities.
• Monitor residents’ comfort levels and promptly address needs and concerns.
• Monitor and record vital signs, weight, and fluid intake/output accurately.
• Report changes in residents’ physical, emotional, or cognitive status to the Caregiver Coordinator or Executive Director.
• Document all care, observations, and resident interactions in compliance with Mosaic policies and regulatory requirements.
