Job summary

Pacific Grove in Forest Grove, Oregon, is seeking a caring Caregiver to support residents in an assisted living and memory care setting. You will provide hands-on care, engage residents in activities, and promote physical, emotional, and cognitive well-being in a compassionate, collaborative team.

Responsibilities include daily living assistance, meal support, health monitoring, and accurate documentation. A high school diploma is required, with caregiving experience preferred and English

Qualifications

• High school diploma or equivalent required.
• Prior caregiving experience preferred.
• Must read, write, and communicate effectively in English.
• Knowledge of assisted living practices and related regulations preferred.
• Ability to work collaboratively in a fast-paced, team-oriented environment.

Responsibilities

• Provide compassionate, hands-on care to residents, including bathing, grooming, dressing, toileting, and mobility.
• Assist residents with meals, snacks, hydration, and feeding as needed.
• Support residents with ambulation, positioning, and range-of-movement activities.
• Monitor residents’ comfort levels and promptly address needs and concerns.
• Monitor and record vital signs, weight, and fluid intake/output accurately.
• Report changes in residents’ physical, emotional, or cognitive status to the Caregiver Coordinator or Executive Director.
• Document all care, observations, and resident interactions in compliance with Mosaic policies and regulatory requirements.
