Job summary

CareLinx by Sharecare is recruiting experienced caregivers to provide non-clinical, in-home care to health plan members and private clients in your area in California. You set your availability in the CareLinx app, and our team matches you with clients that fit your schedule and goals.

Once onboarded, you’ll work with the CarePartner team to find the right matches based on location, client needs, and your availability.

• Able to follow care plans and document tasks accurately.

Responsibilities

• Assist clients with activities of daily living (ADLs): companionship, meal prep, mobility support, bathing/grooming/dressing, medication reminders (non-clinical), and light housekeeping.
• Complete and document all care tasks accurately in the CareLinx mobile app during each shift.
• Monitor and report changes in a client’s condition or self-care abilities to the CareLinx clinical team.
• Submit timesheets on time through the CareLinx app.
• Communicate with your CarePartner regarding any case updates or client changes.
• Ensure patient privacy, dignity, and well-being in a safe and secure environment.
• Maintain client confidentiality at all times.
• Additional shift opportunities (based on client needs): transportation and errand support; hospital discharge escort; telehealth navigation.

WHAT YOU'LL DO

Core responsibilities for all caregivers:

WHAT WE'RE LOOKING FOR

Required qualifications:

Minimum 6 months ofcaregiving experience, OR relevant education/certification (CNA, HHA, MA, or equivalent)

Accessto a smartphone capable of running theCareLinxapp (requiredfor scheduling, documentation, and timesheets)

Ability to communicate in English (bilingual caregivers strongly encouraged to apply)

Reliable transportation to and from client homes

Must pass a background check

Program-specific requirements (may apply depending on assignment):

Completion of program-specific training prior to accepting shifts

Medical clearance such as TB test, physical exam, or drug screening

Valid driver's license and MVR check if accepting driving shifts

Additionaldocumentation may berequiredbased on state law or client contract
