Job summary

CareLinx by Sharecare is actively seeking experienced in-home caregivers to provide non-clinical support to health plan members and private clients. You will use the CareLinx app to manage availability, document tasks, and connect with clients based on location and individual needs.

Join the CarePartner team to build consistent schedules, stay engaged through the app, and communicate proactively with clients. A background check, English proficiency, and a smartphone are required to begin.

Qualifications

• Minimum 6 months caregiving experience, OR relevant education/certification (CNA, HHA, MA, or equivalent).
• Access to a smartphone capable of running the CareLinx app (required for scheduling, documentation, and timesheets).
• Ability to communicate in English (bilingual caregivers strongly encouraged to apply).
• Reliable transportation to and from client homes.
• Must pass a background check.
• Program-specific training prior to accepting shifts may apply.
• Medical clearance such as TB test, physical exam, or drug screening may be required.
• Valid driver's license and MVR check if accepting driving shifts.

Responsibilities

• Assist clients with activities of daily living (ADLs): companionship, meal prep, mobility support, bathing/grooming/dressing, medication reminders (non-clinical), and light housekeeping.
• Complete and document all care tasks accurately in the CareLinx mobile app during each shift.
• Monitor and report changes in a client’s condition or self-care abilities to the CareLinx clinical team.
• Submit timesheets on time through the CareLinx app.
• Communicate with your CarePartner regarding any case updates or client changes.
• Ensure patient privacy, dignity, and well-being in a safe and secure environment.
• Maintain client confidentiality at all times.
• Additional shift opportunities based on client needs: transportation and errand support; hospital discharge escort; telehealth navigation.

WHAT YOU'LL DO

Core responsibilities for all caregivers:
