What You Will Do:

You will implement personalized care plans that support the whole person—mind, body, and spirit. Your role involves treating clients like family and enhancing their quality of life.

Essential Functions:

• Deliver exceptional care in clients' homes
• Assist with personalized weekly schedules and care plans
• Complete daily tasks safely and professionally (ADLs)
• Provide companionship and emotional support
• Assist with personal care, mobility, incontinence, housekeeping, feeding, and meal prep
• Maintain safety, order, and client enjoyment
• Transport clients to appointments as needed
• Document tasks and report changes in clients' conditions
