Job summary

The Society of St. Vincent de Paul - Phoenix is seeking a Workforce Case Manager to support guests facing homelessness or at risk of homelessness. You will manage caseloads, develop individualized service plans, and connect clients with housing, employment, and education resources.

You will work within a team, communicate thoroughly, and coordinate with partners to remove barriers to progress, while maintaining organized records and a client-centered approach.

Qualifications

• Minimum 1+ years experience in Case Management (paid or internship) OR minimum 2+ years of progressive experience in human services role
• Experience working with vulnerable populations in shelter, transitional housing, or similar environments preferred
• Trauma-informed and strengths-based support approach desired

Responsibilities

• Carry a caseload of workforce participants, meeting with each resident at least weekly.
• Actively recruit new participants and assess eligibility for workforce programs.
• Develop assessments and individualized service plans to identify needs, strengths, goals, and barriers.
• Monitor and document resident progress toward stabilization, income, and housing goals.
• Maintain timely and accurate records in data systems within 24 hours of interaction.
• Coordinate incident intervention and conflict resolution with staff and security.
• Connect clients with resources such as skills training, education, and certifications.
• Coordinate with employers, training providers, and community organizations for job placement opportunities.
• Attend and participate in staff meetings, case conferences, trainings, and agency events.
• Contribute to SVdP’s Culture of Invitation by engaging volunteers and supporting their involvement.
• Other duties as assigned by program management.
