Job summary

Qualifications

• Graduate of an approved school of nursing, therapy or social work.
• Must be licensed in the state where they currently practice.
• Two years' demonstrated field experience.
• Knowledge of federal, state, and local laws governing Medicare home health and hospice.
• Basic technology skills, including operation of a mobile device.

Responsibilities

• Assists patients in navigating post-acute care with the goal of impacting outcomes and satisfaction.
• Integrates clinical guidelines and protocols to develop patient-centered transition plans.
• Represents the area branches in strategic relationships with health systems and physicians.
