Job summary

Fairview Range is hiring a Social Worker (BSW) for our Care Transitions Department in Hibbing, MN. You will provide discharge planning tools and coordinate resources to ensure seamless transitions of care for patients and families.

The role requires a bachelor’s degree in social work, MN social work licensure, and a collaborative spirit to support a multidisciplinary team while maintaining patient confidentiality and high-quality care.

Qualifications

• Bachelor's degree in social work from an accredited program.
• Licensed Social Work current in MN.
• Master of Arts in social work preferred (MA).

Responsibilities

• Demonstrate patient-centered and customer focus by treating patients/customers as top priority to meet or exceed needs and expectations.
• Engage in monitoring and evaluating feedback from patients and customers to improve service.
• Coordinate referrals to internal or community resources.
• Collaborate with the multidisciplinary team to develop and evaluate the patient's treatment plan.
• Coordinate patient discharge planning process.
• Maintain patient confidentiality and comply with policies and laws.
