Job summary

Mille Lacs Health System in Minnesota is seeking a Hospital Social Worker (LSW/LMSW) to provide comprehensive case management, discharge planning, and supportive counseling for patients with acute to terminal conditions within an interdisciplinary team.

You will assess psychosocial needs, coordinate referrals, document thoroughly, and engage with families and community resources to support safe transitions of care.

Qualifications

• Bachelor's degree in Social Service field required.
• Must be a licensed Social Worker in Minnesota.
• Master's degree in Social Work preferred with 5 years mental health experience.
• Knowledge of EMR and standard computer tools.

Responsibilities

• Provide comprehensive case management and discharge planning.
• Conduct psychosocial evaluations and organize referrals to community resources.
• Coordinate with interdisciplinary team and communicate with families.
• Maintain timely, accurate progress notes and documentation in EMR.
• Engage in education, outreach, and program development activities.
