Responsibilities

• Provide discharge planning tools and coordinate resources for seamless transitions of care.
• Counsel individuals, families, and groups to alleviate emotional distress and support adaptive behavior.
• Collaborate with the multidisciplinary team to develop and evaluate treatment plans.
• Coordinate patient discharge planning process and referrals to internal or community resources.
• Maintain patient confidentiality and comply with policies, laws, and standards.
