Job summary

Nebraska Medicine in Omaha is seeking a nurse to coordinate a patient-centered, interdisciplinary discharge planning plan for patients in the acute inpatient setting. You will work with internal and external care providers to ensure a seamless transition of care and arrange post-acute transfers or in-home support services.

The role requires at least three years of nursing experience, a BSN, strong communication, and the ability to exercise independent clinical judgment while adhering to policies

Qualifications

• Minimum of three years nursing experience required.
• Bachelor’s degree in nursing (BSN) required.
• Effective verbal and written communication skills required.
• Ability to prioritize and make independent clinical judgments required.
• Experience with basic patient care equipment, policy procedure and protocol manuals required.
• Ability to work collaboratively and cooperatively with an interdisciplinary team required.
• Effective critical thinking/analytical skills required.
• Ability to effectively evaluate care across the continuum required.
• Current Nebraska RN license or compact state license required.
• BLS certification from an AHA-endorsed program required.

Responsibilities

• Coordinate a patient-centered interdisciplinary plan of care for patients in the acute inpatient setting.
• Collaborate with internal and external care providers, facilities and partners to ensure a coordinated post-acute discharge or treatment plan.
• Implement workflows and processes facilitating discharge care planning activities such as post-acute facility transfers or in-home support services.
