Qualifications

• Graduate of an accredited school of nursing.
• Minimum two years of acute hospital clinical experience.
• RN license in NE (Nebraska).
• BLS/CPR certification required within 90 days.

Responsibilities

• Completes discharge planning assessment and documents findings.
• Develops a multidisciplinary discharge plan with the team and patient.
• Oversees and evaluates implementation of the discharge plan.
• Collaborates with care teams to ensure appropriate inpatient resource use.
• Facilitates timely transitions to post-acute services based on needs.
• Identifies readmission risk and intervenes for at least 30 days.
