Job summary

Bryan College Of Health Sciences is seeking a Care Transitions RN to coordinate clinical, psychosocial, and utilization aspects of the patient’s hospital journey, ensuring smooth transitions and appropriate levels of care. You will work with the interdisciplinary team across acute rehab, ED, and utilization management to support discharge planning and care coordination.

The role requires active Nebraska RN licensure, 2+ years of clinical experience, and CPR certification, with preferred

Qualifications

  • Current RN licensure in Nebraska or approved compact state as defined by the Nebraska Nurse Practice Act.
  • Minimum of two years recent clinical experience required; prior care coordination/utilization management experience preferred.
  • Basic Life Support (CPR) certification required.

Responsibilities

  • Create and update personalized transitional care plans for patients.
  • Perform transition planning activities at onset and throughout hospitalization.
  • Collaborate with patient’s provider and care team to manage length of stay and level of care for transitions.
  • Identify need for supportive counseling and refer to appropriate provider.
  • Participate in utilization management, payer practices, and regulatory compliance.
  • Promote quality improvement initiatives and health care outcomes.
