Job summary

You will assess, plan, and facilitate care while collaborating with physicians, nursing, insurers, and post-acute providers to optimize outcomes and experience; strong clinical, communication, and advocacy skills are essential.

Qualifications

• Graduate of an accredited school of nursing.
• Minimum two (2) years of acute hospital clinical experience.
• Master's in Case Management or Nursing field with 1 year of experience.
• Registered Nurse license in NE.
• Basic Life Support (CPR) within 90 days.

Responsibilities

• Completes and documents a discharge planning assessment.
• Facilitates development of a multidisciplinary discharge plan.
• Oversees and evaluates the implementation of the discharge plan.
• Collaborates with the multidisciplinary team to ensure progression of care and appropriate resource use.
• Coordinates transitions to post-acute services based on clinical needs.
• Identifies readmission risks and implements mitigation strategies for at least 30 days.
