Qualifications

• Graduate of an accredited school of nursing with RN licensure.
• Minimum two years of acute hospital clinical experience.
• Masters in Case Management or Nursing preferred; RN licensure required.
• CPR/BLS certification within 90 days.

Responsibilities

• Completes and documents discharge planning assessments and updates plans as needed.
• Facilitates development of multidisciplinary discharge plans involving relevant health team members.
• Oversees and evaluates the implementation of the discharge plan.
• Collaborates with the healthcare team to ensure progression of care and appropriate inpatient resource use.
• Coordinates with post-acute providers to ensure timely transitions to appropriate services.
• Identifies readmission risk and implements mitigation strategies for 30 days.
