Job summary

Enhabit Home Health & Hospice is seeking qualified clinicians to support patients in post-acute care, improving outcomes and referral satisfaction. The role emphasizes adherence to evidence-based guidelines and collaboration with health systems and physicians.

Ideal candidates have nursing, therapy or social work backgrounds, are licensed, and bring field experience. A valid driver's license and reliable transportation are required to serve patients in the Pittsburgh area.

Qualifications

Must graduate from an approved school of nursing, therapy or social work.
Must be licensed in the state where they currently practice.
Two years of demonstrated field experience.
Knowledge of federal, state, and local laws and regulatory guidelines for Medicare home health and hospice operations.
Basic technology skills, including use of a mobile device.

Responsibilities
