Job summary

Nashua Post Acute and Healthcare Center is seeking a Clinical Liaison to build strong relationships with hospitals and healthcare partners, facilitating seamless care transitions and referrals. The role requires travel between facilities and strong communication within a multidisciplinary team.

The ideal candidate holds an RN, LPN, or LSW license (preferred) with an associate degree (bachelor's preferred) and a valid driver's license.

Qualifications

• RN, LPN, or LSW licensure preferred.
• Associate's degree required; Bachelor's degree preferred.
• Valid driver's license and travel between facilities essential.
• Strong communication and relationship-building skills.

Responsibilities

• Build and maintain relationships with medical professionals, hospitals, and healthcare facilities to support effective care coordination and referrals.
• Promote the organization's philosophy, mission, and objectives with internal and external stakeholders.
• Attend facility meetings, calls, and in-services to support collaboration and communication across care settings.
• Demonstrate honesty, integrity, and professionalism in all interactions; safeguard patient information and hospital property.
• Maintain accurate referral-tracking logs to support admissions and coordination.
• Prepare and distribute timely end-of-day reports to department leaders for alignment.
• Collaborate with interdisciplinary teams to ensure smooth transitions of care and address resident needs.
