Job summary

Newton-Wellesley Hospital, part of Mass General Brigham, is seeking a Care Transition Specialist to perform administrative duties that support care progression and patient transitions. You will collaborate with nurses, social workers, physicians, and the care team to advocate for patients and families.

Qualifications

• High School Degree or GED required.
• Associate's Degree or Bachelor's Degree preferred and health care experience preferred.
• Preferred experience in hospital discharge planning, long term care facility, community health or utilization review.

Responsibilities

• Provide direct administrative support to the care team, patients, and patients' caregivers.
• Coordinate post-discharge tasks, appointments, equipment requests, and medication authorizations.
• Collect, confirm, and verify key patient information; assist with transportation coordination.
• May participate in family meetings and interdisciplinary huddles as needed.
