Job summary

Mass General Brigham is seeking a Case Management Resource Specialist to support RN Case Managers with hospital discharge planning. This patient-facing and administrative role coordinates services such as rehab, home care, and medical equipment.

A Bachelor’s degree is strongly preferred; 1–3 years healthcare experience is common. Schedule: Mon–Fri with 1 weekend day/month and 2 holidays; hours typically 8:30 AM–5:00 PM.

Qualifications

Responsibilities

• Research community resources and opportunities
• Maintain brochures and information packets for resources
• Collaborate with social work staff to obtain information
• Assist in planning in-service presentations on resources
• Assess patient/family understanding of available resources
• Educate patients/families about resources and how to access them
