Qualifications

Responsibilities

• Provides direct administrative support to the care team, patients, and caregivers (post-discharge tasks, forms, scheduling).
• Collects, verifies and documents key patient information (demographics, consent, benefits).
• Maintains knowledge of post-acute resources and refers patients to community services.
• Arranges patient transportation under the care team guidance; participates in huddles as needed.
