Mass General Brigham's Newton-Wellesley Hospital is seeking a Care Transition Specialist to support care progression and transitions across the continuum. This per diem, Saturday-only role collaborates with nurses, social workers, physicians, and families to advocate for patients and ensure high-quality care and satisfaction.

The position emphasizes gathering and verifying patient information, coordinating post-discharge tasks, arranging transportation, and documenting activities for regulatory

Qualifications

• Interpersonal skills to interact effectively with various levels of staff, patients, families and community organizations.
• Extensive knowledge of regulations, community organization, state and federal systems, medical terminology and levels of health care.
• Must be able to manage a variable workload with the ability to constantly change priorities.
• Requires ability to work proactively and independently.
• Requires basic typing and/or computer data entry skills, experience with personal computer and software desirable.
• Must be very flexible in a constantly changing environment.

Responsibilities

• Provide administrative support to the care team, patients, and caregivers for post-discharge tasks.
• Assist with ordering equipment, forms, and medication authorizations.
• Distribute forms to comply with regulations.
• Arrange patient transportation as directed.
• Participate in family meetings and interdisciplinary huddles.
• Complete administrative documentation for the care team.
• Collect and verify patient demographics, proxy, and preferences.
• Maintain resources about post-acute care and community services across the continuum.
• Refer patients to community services.
• Collaborate with social agencies, clinics, schools, and courts.
