Job summary

Mass General Brigham is seeking a Care Transition Specialist for Newton-Wellesley Hospital, working Saturdays 8:30am-4:00pm to support care progression across the continuum. The role collaborates with nurses, social workers, physicians, and the care team to advocate for patients and families while ensuring high-quality care and resource management.

Responsibilities include handling post-discharge tasks, coordinating transportation, and managing forms while maintaining regulatory compliance.

Qualifications

• Education: High School Degree or GED required; Associate’s or Bachelor’s degree preferred with health care experience.
• Preferred experience in hospital discharge planning, long-term care facility, community health or utilization review.

Responsibilities

• Provide direct administrative support to the care team, patients, and patients’ caregivers.
• Support administrative tasks related to post-discharge care (e.g., 4Next process, faxing to long-term care facilities, follow-up appointments).
• Assist with ordering of equipment, completion of forms, and medication authorizations.
• Distribute key forms to comply with regulations (e.g., MOON, IM).
• Arrange all types of patient transportation under the care team guidance.
• Participate in family meetings and interdisciplinary huddles as needed.
• Complete administrative documentation under the direction of the care team.
