Job summary

Soleo Health seeks a Clinical Transition Coordinator to partner with physicians, case managers, and operations to transition patients from hospital or facility to an alternate site of care in the San Francisco Bay area.

You will educate patients/families, secure new referral sources, and coordinate with the healthcare team to ensure smooth transitions and payer documentation. Monday–Friday 8:30am–5pm; some weekends may be required.

Qualifications

• Bachelor's degree or medical background preferred.
• Minimum five (5) years of clinical experience, home health field or management experience preferred.
• Knowledge of hospitals, skilled nursing/rehab facilities including case management and discharge planning service needs.
• Advanced practice expertise to identify and implement improvement processes, and the ability to design, direct and implement health care plans on behalf of the patient.

Responsibilities

• Territory: Fremont, San Jose, San Francisco, Mt View, Salinas.
• Sales Support/Patient Referral: strategize and secure new referral sources in geographic market; collaborate with territory manager to gain referrals from physicians, case managers, and other points of contact.
• Patient Coordination: work with multidisciplinary team to maintain and implement updated care plans; communicate with healthcare team; document demographics, clinical, and payer information; assist with paperwork and signatures for payer reimbursement.
• Nurse Liaison: communicate with referring physician’s office and hospital case management; discuss infusion services and outcomes.
• Patient/Family Education: conduct pre-discharge hospital visits to determine need and eligibility; educate patients on services and payer responsibilities; work with marketing and sales to educate physicians/facilities.
• Administrative: maintain and submit expense reports, referral tracking, territory reports, and training documents.
