Soleo Health is seeking a Clinical Transition Coordinator to support the San Francisco, CA area. The role involves coordinating patient transitions from hospital to alternative sites of care, educating patients, and securing new referral sources in collaboration with the territory manager.

The position requires a bachelor’s degree or medical background and at least five years of clinical experience, preferably in home health.

Qualifications

• Minimum five years of clinical experience, home health field or management experience preferred.

Responsibilities

• Territory: Fremont, San Jose, San Francisco, Mt View, Salinas.
• Sales Support/Patient Referral: strategize and secure new referral sources with Regional VP and sales team.
• Patient Coordination: coordinate a coordinated care plan with multidisciplinary team for transitions to alternate sites of care.
• Nurse Liaison: liaise with hospital case management and referring physicians to discuss infusion services and outcomes.
• Patient/Family Education: educate patients and families on services, payer responsibilities and eligibility.
• Administrative: maintain and submit expense, referral tracking and territory reports.
