Soleo Health is seeking a Clinical Transition Coordinator for the San Francisco, CA area. This role collaborates with physicians, case managers, and operations to coordinate patient care transitions from hospital to alternative sites of care.

Responsibilities include securing new referrals, coordinating with the care team, educating patients and families, and ensuring payer documentation is complete and accurate. Strong clinical background and five years of experience are preferred.

Qualifications

• Bachelor's degree or medical background required.
• Minimum five years of clinical experience preferred.
• Knowledge of hospitals, skilled nursing/rehab facilities, case management and discharge planning.

Responsibilities

• Coordinate patient transitions from hospital to alternate care sites.
• Develop and secure new referral sources with physicians and care teams.
• Educate patients and families about illness, treatments, and payer responsibilities.
• Liaise with physicians, case managers, and operations to streamline care transitions.
• Maintain documentation for referral tracking, paperwork, and payer submissions.
• Assist with paperwork and signatures needed to commence therapy and submit for reimbursement.
