• BLS certification required.

Responsibilities

• Coordinate discharge planning and transfer clients to post-acute settings.
• Collaborate with healthcare team to implement transition of care plan.
• Document all interventions in the patient record and discharge plan.
• Follow up with patients and providers regarding post-acute needs.
• Serve as liaison to community resources and maintain clear discharge communication.
