Job summary

Tucson Medical Center is seeking a Coordinator Patient Care to support case management and discharge planning, coordinating post-discharge resources, transportation, and follow-up appointments.

You will document patient preferences in EPIC, collaborate with case management and admission teams, and ensure compliance with Medicare notices; strong data handling and communication with physicians and staff are essential.

Qualifications

• Associates degree or health care related certification preferred.
• Two years of related healthcare experience preferred.
• Quality metrics and data analysis experience preferred.

Responsibilities

• Identify and discuss patient discharges with case management team daily and coordinate follow up appointments, outpatient appointments and/or DME needs.
• Coordinate patient transportation to next level of care using cost-effective options; collaborate with case management coordinators.
• Ensure timely delivery of Medicare IMM and MOON notices and related regulatory communications.
• Collaborate with admitting and utilization teams on delivery of required Medicare notices.
• Participate in creating standard work for regulatory processes; manage DME loan closets and review post-acute preferences in EPIC.
• Exhibits excellence in customer service and adheres to TMCH values in interactions with patients, visitors and staff.
