Qualifications

• BLS certified, or ability to obtain within 6 months of employment.
• Must demonstrate effective communication and customer service skills, human relation skills and time management skills.

Responsibilities

• Assist in communicating the discharge plan to patients and families.
• Facilitate safe and timely transitions of care from acute care to appropriate post-acute settings.
• Collaborate with the care transitions team to coordinate on-site and telephonic discharge arrangements.
• Provide face-to-face interactions with patients and families to support planning.
• Identify barriers to timely discharge and ensure clear communication of the care plan.
• Coordinate with external entities and post-acute vendors to arrange services.
