Job summary
Banner North Colorado Medical Center is seeking a Transition Care Assistant to guide patients and families through discharge planning and post-acute care coordination. You will work with physicians, nurses, nutritionists, and pharmacists to ensure seamless transitions and provide crucial support throughout the process.

- Bachelor’s degree in social work or related field, or a licensed nurse/therapist.
- Knowledge of government/community resources (Medicare/Medicaid).
- Strong communication, customer service and time management.
Responsibilities
- Guide discharge planning and transition of care with the care team.
- Review daily holdovers and participate in multidisciplinary rounds.
- Communicate discharge plans and provide family support.
- Document interventions in the medical record and coordinate post-acute care.
- Perform follow-up calls and connect patients with community resources.
- Maintain knowledge of Medicare/Medicaid benefits for transition planning.
