Job summary

Providence at Home with Compassus seeks a Care Partner to coordinate safe, efficient transitions of care for hospitalized patients and evaluate appropriate post-acute home-based services. Qualifications

• Associate's degree required.
• Bachelor's degree preferred.
• 2–3 years healthcare experience preferred.

Responsibilities

• Evaluate patients for home-based and post-acute care services based on clinical and environmental factors.
• Coordinate timely discharge planning and transition-of-care plans.
• Educate patients and families on post-acute care options and services.
• Maintain relationships with physicians, case management, nursing teams and discharge planners.
• Identify patients appropriate for hospice or GIP level of care and coordinate assessments.
• Document all care coordination activities in the EMR and manage referrals in systems.
• Support post-acute network performance and patient outcomes through quality initiatives.
