Job summary

State of Franklin Healthcare Associates is seeking a qualified Medical Assistant to support patient transitions from inpatient to home or post-acute settings. You will develop and monitor care plans, use motivational interviewing to improve outcomes, and document thoroughly in CareTrac.

The role involves identifying social determinants of health, coordinating resources with social workers, and assisting providers to close care gaps through proactive outreach. A CMA/RMA credential is required.

Qualifications

• Must be a graduate from a recognized certified and/or registered medical assistant program or an approved school of nursing.

Responsibilities

• Help patients transition from an inpatient setting back home or to a post-acute setting.
• Development, initiating, and monitoring of a plan of care for the patient self-management.
• Utilize knowledge of motivational interviewing techniques to enhance patient outcomes.
• Access, plan, implement, monitor, and evaluate options and services required to meet patients’ needs.
• Identify social determinants of health and refer patient to community resources and social worker support.
• Accurately document in the medical record and CareTrac.
• Assist providers with closing care gaps through patient outreach.
