Job summary

The JOY FM is seeking a Certified Medical Assistant (Care Manager) to deliver mostly telephonic care management. You will manage transitional care after hospital or facility discharges and support medication adherence through timely outreach.

In this role you will document every patient interaction, coordinate with providers and the care team, and address care gaps. A Georgia CMA or LPN license and strong communication are required for success.

Qualifications

• Active CMA or LPN licensure in Georgia.
• Strong phone communication and patient-centered approach.
• Proficient typing and real-time documentation.
• Ability to work independently in a structured, phone-based environment.

Responsibilities

• Manage transitional care after hospital or facility discharge.
• Support medication adherence by reviewing regimens with patients and coordinating with the care team.
• Deliver chronic care management (CCM) services through regular check-ins on patients with ongoing conditions.
• Document every patient interaction thoroughly in our care management system.
• Coordinate with providers and practice staff to address patient needs and close care gaps.
