Job summary

BrightSpring Health Services is seeking a Care Transition Coordinator to facilitate patient transitions from healthcare facilities to home health or hospice care. This pivotal role involves evaluating patient eligibility, coordinating care plans, and ensuring services are arranged according to patient needs.

The ideal candidate will have at least one year of experience in home health or hospital-based case management and hold a current RN, LPN, SW, or PT license. Responsibilities include conducting patient education, collaborating with healthcare providers, and managing sales-related administrative functions.

Qualifications

• Minimum of one (1) year of experience in home health or hospital-based case management.
• Preferred: One (1) to three (3) years of experience in medical marketing or healthcare business development.
• Must possess a valid driver’s license and reliable transportation.

Responsibilities

• Achieve monthly personal production goals for assigned locations.
• Evaluate patients and physician orders for home care eligibility.
• Conduct face-to-face patient transitions and provide agency education.

Skills

Minimum one year of experience in home health or hospital-based case management

Understanding of home health eligibility criteria

Medical marketing or healthcare business development

Strong clinical knowledge in nursing or therapy

Education
