Isaiah House seeks a dedicated Discharge Coordinator in Kentucky to help clients transition from inpatient care to continued recovery support. You will coordinate aftercare placements, monitor discharge timelines, and collaborate with Clinical, Programs, and Operations teams to ensure smooth handoffs.

The role emphasizes relationship-building with families and referral sources, with a focus on accurate documentation and timely communications throughout the discharge process.
