Job summary

Platinum Home Health Care is seeking a Lead Care Coordinator to drive daily staffing operations, optimize caregiver placement, and ensure timely case coverage. You will mentor Scheduling Coordinators, provide clear updates, and maintain high productivity and patient satisfaction.

The role emphasizes staffing performance, process adherence, and proactive communication to support client retention and service quality.

Qualifications

Minimum of three (3) years of experience in home care coordination, staffing, or scheduling.
Lead or senior coordinator role experience preferred.
Proficiency in Google Suite and HHAeXchange or similar scheduling/EVV systems.
Ability to manage multiple cases in a fast-paced, high-volume environment.
Strong understanding of home care regulations and NYS DOH requirements.
Ability to motivate and guide a team while maintaining productivity and accountability.
Excellent communication and interpersonal skills; professional relationships essential.
Strong organizational skills and task prioritization to meet deadlines.

Responsibilities

Staffing Resource Utilization: ensure open cases are staffed using available resources and outreach tools.
Team Motivation & Communication: provide updates on cases, staffing priorities, and coverage gaps; timely team updates every 2 hours.
Operational Support & Escalation: identify staffing gaps, elevate issues to Care Manager, maintain continuity of care.
