Job summary

Primecare Home Care Services Inc. in Macon, GA seeks a Care Relationship Manager to oversee an assigned caseload, ensure services are authorized, and schedule caregivers to meet client needs.

You will serve as primary contact for clients and caregivers, coordinate staffing, monitor service delivery, complete required Health Coach duties for Structured Family Caregiving, and collaborate with clinical and support teams to maintain continuity of care.

Qualifications

• Must meet SFC Health Coach qualification requirements through one of the following: Associate Degree; or 2+ years of experience in community health, long-term care, or chronic disease management.
• Experience managing multiple clients, schedules, or service-related priorities in a fast-paced environment.
• Strong customer service and interpersonal skills with the ability to communicate with clients, caregivers, and internal teams.
• Proficiency with Microsoft Office and ability to learn scheduling, payroll, and client management systems.

Responsibilities

• Manage an assigned caseload of clients, ensuring ongoing service needs and caregiver coverage.
• Staffing, schedule management, service monitoring, and timely follow-up on issues.
• Documentation of client and caregiver contacts, complaints, and incidents; ensure resolution.
• Monitor caregiver time-keeping, attendance, and scheduling discrepancies.
• Coordinate with clinical, operational, and support teams to maintain continuity of services.
