The Ministerial Association of Colusa C invites applications for a Referral Coordinator. This role provides administrative and service coordination support to Care Coordinators, connecting neighbors to health, behavioral health, and social services through referrals, scheduling, and documentation.

You will work with care team members to remove barriers to access, arrange transportation, track referrals, maintain records, ensure HIPAA compliance, and support outreach.

Qualificações

• High school diploma or equivalent.
• Basic computer skills including Microsoft Office, email, and data entry systems.
• Strong organizational skills and attention to detail.
• Experience in healthcare or social services preferred.

Responsabilidades

• Coordinate referrals to healthcare providers, housing resources, transportation, and other services.
• Schedule and confirm appointments with medical providers and social service agencies.
• Provide reminder calls, messages, and follow-ups to ensure attendance.
• Maintain accurate service records and documentation.
• Coordinate transportation for appointments and program activities.
• Track referrals and follow up with providers to ensure services were received.
• Assist neighbors with forms and required paperwork.
• Communicate with Care Coordinators and Community Supports staff for continuity of care.
• Maintain referral tracking systems to ensure compliance.
• Provide administrative support for care coordination activities.
• Support outreach and engagement for appointments and referrals.
• Uphold confidentiality and HIPAA compliance.
