Molina Healthcare is seeking a care coordination support professional to assist with member intake and portal operations. This role monitors the intake support center, ensures access to care coordination information for stakeholders and members, and triages inquiries to the appropriate departments.

Local travel may be required based on state contracts. The ideal candidate has at least 2 years in care management or related fields, an LPN/LVN license if required, strong communication and detail

Qualificações

• 2+ years of experience in care management or a managed care setting.
• LPN/LVN license active if required by state contract.
• Strong customer service and interpersonal skills.
• Excellent attention to detail and organizational abilities.
• Home office with high-speed internet.

Responsabilidades

• Monitor daily intake support center and care coordination portal.
• Ensure stakeholders and members access pertinent care coordination information.
• Triages member inquiries and directs to appropriate departments.
• Serve as point of contact to external care coordination entities and providers.
• Facilitate member referrals as needed.
• Participate in training for care coordination portal use.
• Help monitor reporting requirements to address member needs.
• Support population health management and related programs.
• Coordinate cross-functional work with internal/external partners.
• Identify operational issues and report to leadership.
• Occasionally conduct training, scheduling, and reporting.
• Travel locally as required.
